I have been divorced for 8 years (Cook County) and would like to resume my maiden name. How do I do this?

I do not want to incur anymore cost as I think I have already paid enough to the systems and attorneys etc. So, is there a way to just go back into court and ask the judge to allow me to start using my name given to me at birth? I declined taking on my name in the last day of our trial but was given the option to take my name back any time in the future. I just don’t know how to do this. Anyone?

Actually it is very simple, I did it. I asked my divorce attorney how to do it and he filled out (for free by the way) a form which had my divorce case number on it which petitioned the court for a name change. I took it my self to the county court clerks office with a self addressed stamped envelope. She had the judge sign it, put the county seal on it and mailed it to me. I took it to the DMV and Social Security office had my name changed then contacted my banks and business acquaintances.
